As a Youth Support Worker | Youth Care Professional, you will play a vital role in providing direct care, guidance, and therapeutic support to adolescents in a structured residential setting. Hope Homes – Holyoke House provides individualized care and support to residents ages 14–21, helping young people navigate challenges, develop essential life skills, and work toward positive futures. Your responsibilities may include: Youth Care & Daily Support • Provide direct care and support to up to five residents, • Supervise daily activities and routines, • Assist with meal preparation, household responsibilities, and transportation to appointments and activities, • Encourage positive decision-making and personal growth, • Build meaningful relationships with residents through consistency and support Therapeutic Support & Collaboration • Assist with the development and implementation of individualized treatment plans, • Support behavior management strategies and therapeutic interventions, • Collaborate with team members to provide consistent and effective care, • Participate in staff meetings, trainings, and professional development activities, • Respond to challenging situations with patience, professionalism, and understanding Documentation & Program Responsibilities • Maintain accurate and complete resident records, • Complete progress notes and incident reports, • Follow confidentiality standards, policies, and procedures, • Communicate important resident updates with the treatment team Who We're Looking For You may be a great fit if you are: ✔ Compassionate ✔ Patient and Understanding ✔ Responsible ✔ Team-Oriented ✔ Strong Communicator ✔ Dedicated to Helping Youth Qualifications Candidates must meet one of the following: • High school diploma or high school equivalency diploma and one year of experience working with individuals with Intellectual/Developmental Disabilities (I/DD) in a group setting OR • Associate or bachelor’s degree from an accredited college or university in a field unrelated to social work or psychology and six months of experience working with individuals with I/DD in a group setting OR • Associate or bachelor’s degree from an accredited college or university in social work, psychology, or a related field Additional Preferred Qualifications The ideal Youth Support Worker: • Understands child development, behavior management, and therapeutic techniques, • Can work independently and as part of a team, • Demonstrates strong communication, problem-solving, and organizational skills, • Has a valid driver’s license and reliable transportation ????
