Job DescriptionMSW Social Worker Schedule: Monday-Friday, 9:00 AM - 5:00 PM Position Type: Full-Time Position Overview We are seeking a compassionate and organized Master's Level Social Worker (MSW) to support clients through comprehensive assessments, care coordination, and advocacy. This role involves managing an assigned caseload, conducting in-person visits in client homes, facilities, and community settings, and collaborating with internal teams and external providers to ensure clients receive appropriate support and services. The ideal candidate will have experience working in a hospital or nursing home environment and the ability to navigate complex client needs with professionalism and empathy. Responsibilities • Manage an assigned caseload and maintain regular communication regarding client needs and progress, • Conduct in-person visits with clients in homes, facilities, and community-based settings to assess overall well-being and safety, • Evaluate client environments and identify potential risks or areas requiring additional support, • Assess client needs, including home care, community resources, and supportive services, and communicate recommendations for coordination with approved vendors, • Monitor ongoing client situations and provide timely updates, documentation, and reports regarding status, risks, and outcomes, • Collaborate with internal teams to ensure services are appropriately arranged and implemented, • Connect clients with external medical, psychiatric, and specialized providers as needed (no direct treatment provided), • Maintain accurate, detailed, and timely case notes in compliance with agency policies, regulations, and ethical standards, • Prepare reports and provide court testimony when required, including clear documentation of client conditions and safeguarding efforts, • Ensure all activities comply with agency procedures, legal requirements, and professional social work standards, • Remain flexible and responsive to urgent client situations and emergencies as needed Required Qualifications • Master's Degree in Social Work (MSW), • Prior experience working in a hospital, nursing home, or healthcare setting, • Ability and willingness to conduct in-person client visits in homes, facilities, and community environments, • Strong assessment, documentation, and case management skills Preferred Qualifications • Excellent organizational skills with the ability to manage multiple cases and priorities, • Strong interpersonal and communication skills, • Experience coordinating community resources and support services, • Ability to work independently while collaborating effectively with multidisciplinary teams Schedule • Monday-Friday, 9:00 AM-5:00 PM, • Must be flexible to respond to client emergencies and urgent situations when needed Join a team dedicated to supporting vulnerable populations by providing advocacy, care coordination, and resources that improve client safety and overall quality of life.
