The Case Manager will serve individuals who are mandated to Felony Alternative to Incarceration (Felony ATI) programming through Brooklyn Supreme Court. Reporting to the Clinical Director, the Case Manager is responsible for conducting intakes and assessments, linking clients to a range of wrap-around services, providing comprehensive trauma-informed case management, facilitating psychoeducational groups, compliance monitoring and reporting. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Domestic Violence & Trauma Services Program offers a comprehensive suite of services to survivors of domestic violence with enhanced focus on education and linkage to health & wellness through both individual and group counseling supportive services. CHASI works in collaboration with the Staten Island Family Justice Center to expand reach and capacity. Domestic violence and trauma services are co-located. Services are provided at two different locations, 26 Bay Street Site and the Staten Island Family Justice Center (FJC). The Intensive Case Manager will manage a high-risk caseload with a specific focus on working with complex, intersectional cases of domestic and gender-based violence including, but not limited to, family violence, elder abuse, human trafficking, and sexual violence. The Intensive Case Manager will navigate clients through the many complex systems called into action in service plans, such as behavioral health services, medical services, criminal justice, family court, and child protective services. The Intensive Case Manager will be a strong advocate who can provide ongoing support to clients with the police, prosecutors, housing, shelters, schools, public assistance, and other service providers. Sixty-seven units are dedicated for families and 203 units are for single individuals. The program has two components to assist clients succeed in housing, socials services and operations. Case management staff focusses on removing barriers to maintain medical care and achieve viral suppression by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focuses on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Job Description The Care Coordinator will assist individuals in navigating complex systems to obtain services such as Medicaid benefits, housing assistance, food programs, transportation, and other community-based resources. This role works closely with clients, government agencies, and community partners to ensure individuals successfully access the support they need. Key Responsibilities • Conduct client intake and assess social and healthcare-related needs., • Assist clients with applications for government programs such as Medicaid, SNAP, housing assistance, and other benefits., • Connect clients with appropriate community resources and service providers., • Track referrals and follow up to ensure services are received., • Maintain accurate documentation and case notes in internal systems., • Communicate with government agencies, healthcare providers, and community organizations to coordinate services., • Provide ongoing support and guidance to clients throughout the service process., • Ensure compliance with program requirements and organizational policies. Qualifications • High School Diploma required; Associate’s or Bachelor’s degree in Social Work, Human Services, Public Health, Healthcare Administration, or related field preferred., • Strong communication and interpersonal skills., • Ability to work with diverse populations and individuals in need., • Strong organizational and time-management skills., • Basic computer skills and experience with data entry or case management systems.
