JOB PURPOSE The Continuum of Care Case Manager at TJOP is responsible for providing comprehensive case management services to individuals and families experiencing homelessness. This role focuses on assisting clients in obtaining and maintaining stable housing by connecting them to appropriate resources, developing individualized housing plans, and offering continuous support to promote long-term self-sufficiency. The Continuum of Care Case Manager will collaborate with community partners, landlords, and supportive service providers to ensure clients receive the necessary assistance to achieve housing stability. Through a client-centered approach, the CoC Rapid Re-Housing Case Manager will help mitigate barriers to permanent housing, promote financial independence, and empower individuals to rebuild their lives. This includes participation in up to six Pop Up and Give community events annually. These occur outside of standard business hours and are scheduled on the third Saturday of the month. ESSENTIAL DUTIES AND RESPONSIBILITIES Case Management & Client Services • Manage a caseload of 20–25 clients, securing and maintaining housing while connecting clients to supportive services and resources for rapid stabilization, • Conduct comprehensive assessments to identify clients' service needs and barriers hindering sustainable self-reliance, • Develop individualized action plans with clients that promote self-reliance, outlining achievable goals and strategies focused on housing, health, and other essential needs, • Advocate on behalf of clients to secure necessary benefits, entitlements, and services, • Assist clients in identifying and accessing community resources and support services, • Serve as a liaison between clients and landlords; support households in housing searches, co-identifying safe and affordable units, negotiating lease agreements and utility arrangements, • Establish and maintain an effective and timely referral network, • Work closely with clients to motivate and reinforce independence, self-sufficiency, and self-esteem throughout their journey, • Provide continuous case management by monitoring client progress, reassessing needs, and evaluating plan effectiveness, • Provide immediate support and intervention during client crises; develop safety plans and connect clients to emergency services when necessary, • Conduct monthly home visits and case reviews for all clients to assess progress and address emerging needs, • Assist clients in developing and maintaining budget plans to ensure stable self-reliance within six months, • Provide guidance on financial literacy, budgeting, saving, and debt management, • Collaborate with the workforce case manager to increase household income, • Connect clients with nutrition education support, • Mediate issues that threaten client self-sustainability, • Prepare and submit monthly success stories to supervisor, marketing, and grants manager, • Maintain accurate and comprehensive case notes documenting client status, progress, assessments, service engagement, and relevant correspondence, • Record all services, referrals, recommendations, case notes, and program documentation into HMIS and Salesforce within 1 business day of client contact, • Complete necessary forms and reports following established procedures, • Performs other related duties consistent with the scope and purpose of the position. Growing together. \n \n Position in this function is responsible for the care management and coordination of all high risk high need patients assigned to a specific site, including both in and out patient clinical care. Assists clinicians in making informed decisions with these patients in order to promote better outcomes and smooth transitions of care. Follows the standards of Case Management and acts as the single point of contact for patients and families and providers. Maintains records and data analysis related to high risk cases to report outcomes and ROI. Works collaboratively with all internal and external partners to ensure better clinical outcomes. \n \n As the Complex RN Case Manager you will be responsible for care management and coordination of care for high risk cardiac and congestive heart failure patients. You would be working in the Cardiology department. You will be working closely with the Cardiology Providers, assisting with the CHF clinic, assisting with remote telemonitoring of patients and assist management/providers to set up other clinics/programs that are of interest. The Complex RN Case Manager will have his/her own scheduled patients who come in for visits as well as those patients who will need to be reached telephonically. \n \n Primary Responsibilities: \n\n • Apply case management standards of practice to focus on effective care of high risk high need patients at a specific site\n, • Serve as a patient advocate and resource and provide critical information and recommendations to the rest of the care team\n, • Participates in assessment activities to develop individualized plans of care in coordination with patient, family and providers\n, • Follows patient through various transitions of care to ensure that any gaps are identified and remedied, break down silos and promote efficient health care delivery\n, • Maintains strong knowledge of UM, Case management, community resources and plan benefits to ensure improved outcomes\n, • Works collaboratively with primary care on site staff to ensure patient compliance and adherence to medical plan of care\n, • Assist clinicians in implementing best practices for chronic care and disease management\n, • Follow standard protocols, processes and policies to include but not limited to the following: Medication Refill, Preventative Services, Managing Conditions, and Disease Case Management as signed by department head or designee\n, • Care management and coordinating care for our high risk Cardiology and CHF patients\n, • Knowledge of Milliman criteria and utilizing these criteria to manage CHF patients\n, • Assist Clinicians in implementing best practices for chronic care and disease management\n, • Participates in assessment activities to develop individualized plans of care with patients, family and Providers\n, • Provide patient education on disease processes to help promote self-management and compliance\n, • Responsible for maintaining an active caseload and provide interventions as needed within area of expertise and within their scope of practice\n, • Heart Failure disease management (telephonic and/or in person) will focus on adherence to lifestyle changes, symptoms recognition by patients, developing individualized treatment plans with patients and helping patients with the implementation of the treatment plan\n, • Responsible for ongoing monitoring of patients treatment plans, adherence, if goals are achieved and evaluation of care; Assisting in reducing readmission rates\n, • Exercise sound judgment (Critical Thinking Skills) in evaluating situations and making decisions; notify appropriate staff/providers in situations requiring assistance\n, • Make outbound telephone calls to patients to assess members' current health status.\n, • Provide telephonic nursing support to patients/caregivers related to disease process, signs and symptoms to report, telemonitoring, individual treatment plans, medications and for any additional concerns\n, • Responsible for monitoring and evaluation of electronic data received in the Telemonitoring Software Application from remote biometric monitors located in the patient's home\n, • Responsible for telephonic nursing assessment of biometric alerts, mission information and intervention accordingly within their scope of practice\n, • Maintains monthly statistics generated from current patient case load and telephonic patient case load\n, • Knowledge of relevant computer systems and software applications (e.g., IDX; MS Word; MS Excel; Outlook; Touchworks) or ability to learn new programs\n, • Communicates with customers in a manner that is clear, concise and understandable. Ability to have their own transportation to travel frequently within the Las Vegas Valley. Ability to pass FIT testing and wear protective mask as deemed by SMA in the course of work at clinic.
