Job DescriptionSalary: 50,000 Summary: The successful candidate will provide a full range of on-site, person-centered, strengths-based services to a caseload of approximately 20-25 clients with the objective to help people live fulfilling lives in the community. The Case Manager will be responsible for counseling, client intakes, case management, and counseling and aftercare services. The manager will collaborate with colleagues to meet the needs of their caseload and achieve program success while fulfilling core purpose, value and vision. Through its various programs, BFL provides a broad range of residential and non-residential services to disabled survivors of domestic violence, to the disabled homeless, and support to physically disabled individuals with mental health and/or chemical addiction issues. BFL Apartments (BFLA) is a NY/NY III type supportive housing program. BFLA offers services to vulnerable and needy populations who are survivors of domestic violence, specifically, homeless disabled adults and disabled veterans and families. This program’s facility is located in the South Bronx. There are two buildings: a 70-unit studio apartment building for single adults (SPMI-chronic homeless population A) and a 50-unit apartment building for families (SPMI/MICA-chronic homeless population G). Barrier Free Living is seeking qualified applicants to join our team. The Family Support Coordinator connects tenants with vital resources, promotes parent and family empowerment, and supports efforts to maintain permanent housing. THE OPPORTUNITY Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. BJI seeks Case Managers for the borough’s Supervised Release Program (SRP). Reporting to Supervising Case Managers, Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, Case Managers will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants’ compliance with program requirements. Job Description Position Summary The Bronx Men’s Homeless Shelter is seeking a dedicated and compassionate Case Manager to assist residents in accessing services and resources that support their transition from temporary shelter to permanent housing. The ideal candidate will have experience working within the DHS/OTDA system, possess strong case management skills, and be committed to helping individuals achieve stability and self-sufficiency. Essential Duties and Responsibilities • Conduct initial resident intakes and enter accurate information into CARES within 48 hours, or as required by DHS/OTDA guidelines., • Establish professional and supportive relationships with residents while maintaining confidentiality., • Create, maintain, and update resident case files and documentation., • Develop and update Independent Living Plans (ILPs) on a weekly or biweekly basis and document all progress notes on the same day services are provided., • Complete Psycho-Social Assessments in CARES within the first month of a resident’s stay and update assessments annually or as required., • Schedule appointments with referral agencies and follow up regarding resident participation and progress., • Monitor and document client engagement and progress on a weekly basis., • Serve as a liaison and advocate for residents with external organizations, including educational institutions, healthcare providers, housing agencies, and legal services., • Assist residents in identifying and overcoming barriers to obtaining permanent housing., • Support residents in maintaining active Public Assistance benefits., • Escort residents to appointments when necessary., • Assist residents in obtaining required documentation, including New York State identification cards, birth certificates, and other essential records., • Provide employment support, including resume preparation, cover letter development, job applications, and referrals to employment resources., • Attend staff meetings, trainings, and professional development activities as required., • Perform additional duties as assigned by the Program Director.
