Job DescriptionNow Hiring: LVN for Adult Hospice and Home Health Join our compassionate team and make a difference in the lives of our patients! We are seeking a skilled LVN to deliver high-quality nursing care in patients homes, under the supervision of a Registered Nurse. This role involves collaborating with the healthcare team, patients, and families to ensure the best outcomes. Job Title: LVN Adult Hospice and Home Health Locations: Lower Valley & Upper Valley Job Types: Part-Time, PRN Shifts: Day Shift, Evening Shift, Night Shift, and Weekends as needed Work Schedule: Monday to Friday, Weekends, Evenings as needed Key Responsibilities: • Patient Care: Provide direct nursing care to patients in their homes, ensuring the implementation of care plans in line with physicians' orders., • Assessment & Documentation: Regularly monitor vital signs, track medical conditions, document changes, and maintain accurate health records., • Medication Management: Monitor patients medication usage and report results, ensuring proper administration and dosage., • Collaboration: Work closely with the interdisciplinary team, patients, and their families to assess, implement, and evaluate care plans., • Personal Care: Assist with emotional, personal, and medical support for patients, providing holistic care to meet their needs., • Care Coordination: Evaluate patient behavior, diagnose conditions, and consult with healthcare professionals to improve patient outcomes.Working Conditions:, • Work in patient homes, often in varying environmental conditions (including possible exposure to bodily fluids and infectious diseases)., • Ability to travel locally to patient homes and manage flexible work hours., • Occasional emergency PRN calls and exposure to adverse weather., • Reasonable accommodations may be made for individuals with disabilities.Qualifications:, • Education: Graduate of an accredited school of vocational nursing, • License/Certifications:, • Current state LVN License., • BLS Certification (required)., • Valid Drivers License., • Experience: Minimum of 2 years as an LVN in Clinical, Hospice, or Home Health settings (preferred).Why Join Us?, • Competitive salary and benefits., • Opportunity to work in a rewarding field, making a direct impact on the lives of patients., • Flexibility in work hours with the ability to schedule around personal commitments.How to Apply: Please submit your resume to recruiter@tricountykidscare.com or contact us at 956-444-4800 for further inquiries.
