Park Affordable is located in the immediate vicinity to Maimonides Medical Center (MMC) in Brooklyn and will have a total of 67 units set aside for formerly homeless families. CAMBA, INC. will provide case management services to 32 of the 67 units which will have an HPD (PBV) rental subsidy. Light support social services will be available for the remaining 35 units as tenants voluntarily engage in services. Position: Case Manager Reports To: Program Manager Location: 864 49th Street, Brooklyn, NY 11220 What The Case Manager Does: • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building)., • May act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc., • May schedule appointments for tenant with referral organizations., • May escort tenants to appointments (educational, medical, social service, etc.), • May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants' behalf., • May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • May evaluate actual living conditions of tenants through home visits., • May prepare marketing materials for the program. CAMBA, Inc. is the on-site services provider at Putnam Court. Thirty-four apartments, 58% of the units in the building, will be master leased by CAMBA and will house Population A chronically homeless single adults who are diagnosed with SPMI (seriously and persistently mentally ill) or MICA (mentally III and chemically addicted). POSITION OVERVIEW The Care Management Supervisor is responsible for maintaining the quality and effectiveness of clinical services of the health home programs. The position incorporates direct staff supervision and a range of administrative tasks related to clinical operations and as assigned by the program director. The staff is responsible for maintaining BAHN regulatory compliance, to provide comprehensive leadership and supervision to the health home team of care managers. KEY ESSENTIAL FUNCTIONS • Provide leadership and ongoing supervision to care management staff., • Provide ongoing oversight and ensure quality assurance regarding care management outreach, engagement, and coordination activities., • Review, approve, completed as needed, and sign off on clinical documentation including but not limited to, comprehensive assessments and plans of care., • Manage the overall care management document submission process as it relates to compliance with monthly billing and coordination services., • Educate and provide recommendations for member care coordination activities/services., • Review, assign and manage member enrollment to care management staff., • Adhere to the standards, policies and procedures, and goals of the Health Home program and funding sources., • Responsible for all member related documentation in the assigned portals and compliance in accordance with the Health Home program regulations., • Act as a liaison in coordinating and documenting member care activities with staff, internal and external service providers., • Perform other appropriate duties as requested by administrator and program director, • Participate in staff and organizational meetings as required., • Participate in all programs related auditing., • Work closely with program director on special projects, assessments, and completion on any staff training need etc., • Participate and perform in quality assurance/quality improvement processes., • Provide onboarding training to staff based on health home clinical documentation standards and/or assign by program director., • Conduct ongoing supervision with care managers to reiterate policy and procedure and health homes documentation workflows., • Review potential disenrollment with care management staff to ensure quality disenrollment as well as increase member retention., • Responsible for managing staff timesheet/payroll submission., • Attend internal and external trainings for personal and professional development., • Conduct staffing interviews., • Responsible for conducting case conference as requested., • Evaluate care managers on work performance and complete annual evaluations and development plans., • Responsible for staff timely completion of internal and external required assessments (ex. Comprehensive Assessments, Eligibility and Appropriateness Assessment, plan of care, consents, HARP, etc.)., • Rotation of work phone for the 24 afterhours hotline to obtain client’s information by answering telephone calls, interviewing clients, and verifying information.
