The ideal candidate will have expertise in care coordination, admission reviews, discharge planning, and utilization management, ensuring patients receive appropriate, timely, and cost-effective care. The role requires close collaboration with physicians, interdisciplinary teams, patients, and families to facilitate safe transitions of care and improve patient outcomes. \n Roles & Responsibilities:\n \n • Perform comprehensive inpatient case management, including care coordination, admission reviews, and discharge planning for acute care patients.\n, • Collaborate with physicians, nurses, social workers, and the interdisciplinary team to develop and implement individualized care plans.\n, • Conduct utilization reviews to ensure appropriate level of care, medical necessity, and compliance with payer and regulatory guidelines.\n, • Coordinate post-discharge services, referrals, and community resources to promote safe and timely transitions of care.\n, • Maintain accurate clinical documentation, communicate effectively with patients and families, and monitor patient progress to optimize quality outcomes.\n Skills:\n \n, • Strong inpatient case management, care coordination, and discharge planning skills in an acute care hospital setting.\n, • Experience performing admission reviews, utilization management, and medical necessity assessments.\n, • Excellent patient assessment, critical thinking, and clinical decision-making abilities.\n, • Strong communication and collaboration skills with physicians, interdisciplinary teams, patients, families, and payers.\n, • Proficient in EMR documentation, regulatory compliance, time management, and coordinating post-acute care services.\n Documents Required:\n \n, • Valid NY State RN license\n, • BLS Certified through AHA\n, • Most updated Resume\n, • COVID Card\n \n \n \n Please submit your resume to or Call/text .
