Job Description Summary: Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach. Under this umbrella, Case managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). This position will focus on providing support and services to LGBT youth. Services provided include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Case manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Job Description JOB TITLE: Solution-Based Casework (SBC) Case Planner EMPLOYMENT TYPE: Full-Time LOCATION: Hybrid – 2-3 days in office (Queens Location) REPORTS TO: Solution-Based Casework Supervisor SALARY: $62,166.99 Annually ABOUT AAFSC: For over 30 years, the Arab-American Family Support Center (AAFSC) has proudly served immigrants and refugees. We help children stay safe, families remain together, and newcomers navigate life in New York City. While our doors are open to all, AAFSC has expertise in serving Arab, Middle Eastern, North African, Muslim, and South Asian communities. JOB OBJECTIVE: To provide high-quality case planning, case work, and case management services to at-risk families referred by the Administration for Children’s Services. These families may be experiencing a wide range of challenges, including personal/marital/relationship problems, financial stressors, acculturation issues, family destabilization, and/or parent/child conflict. The Case Planner provides intensive preventive services to avoid foster care placement and ensure the safety and welfare of children in the home. He or she provides the family with services according to ACS standards and requirements. The Case Planner follows a Solution Based Casework (SBC) practice model. SBC is based on three theoretical foundations: family life cycle theory, relapse prevention/CBT theory, and solution-focused family therapy. Partnership with families in each and every case is critical to reducing risks and solutions should utilize prevention skills to achieve those goals. PRIMARY DUTIES & RESPONSIBILITIES: Case Management & Advocacy Ensure timely completion of SBC training & certification. Conduct home visits and office visits to assess child safety and risk elements, complying with ACS requirements. Face-to-face contacts should be conducted twice per month, at a minimum and includes home visits (two home visits must be conducted if children 0-2 years old are present) and office visits (e.g., school, doctor’s office, AAFSC), as well as other out-of-home visits (e.g., park). More visits may be required. Provide case work/case management services to families receiving Preventive Services (12 cases); which includes but is not limited to providing supportive casework, focusing on service goals, and child welfare laws, improving family relationships and bonding, coping skills, parenting, etc. Ensure joint-home visits are conducted within 5 business days from the date of referral. Evaluate and assess the current family functioning including child safety and home safety (e.g., assessing other adults in the home, ensuring safe sleep, working fire alarms, window guards, etc.). Work in partnership with the family to formulate outcomes & service plans with the family. Ensure all initial intake documents (AAFSC intake, Domestic Violence screening, child trafficking) are completed and processed accurately using AAFSC internal database (i.e., Apricot). Advocate for the family by linking them to needed services in the community: medical, mental health, entitlements, day care, home health aide services, substance abuse treatment, anger management, etc. and follow up to monitor participation and progress. In collaboration with the Conference Facilitator, support the facilitation of Family Team Conferences and Service Termination Conferences, as well as other relevant ACS-led conferences. Family Team Conferences must be conducted within 4 weeks of the FASP due date. Ensure FASPs are launched 30 days prior to the due date and submitted to Supervisor 3 weeks prior to the due date. Complete Initial, Comprehensive, all Reassessment FASPs, and Plan Amendments for all Advocate Preventive Only (ADVPO) cases offline. Complete and Submit Child Trafficking Screening for Child Welfare Services and ADVPO cases. Evaluate walk-ins and community referrals for needed services and potentially for advocacy cases. Provide case coverage in the absence of a co-worker (as needed). Schedule and coordinate monthly visits with FSU workers (applicable to FSU cases only). Quality Assurance, Communication, and Documentation Complete all documents required by ACS in a timely manner (5 business days) including but not limited to progress notes and FASPs using templates in a manner that reflects the progress or lack thereof, including the reaction of the family to services during the review period or contact. Complete all ACS and internal forms required for case opening and closing as well as obtain consents and forms needed for other case activities. Obtain and maintain collateral contacts with all other service providers including school and medical reports on a regular basis. Comply with all data entry requirements for database systems including inputting information in a timely manner (PROMIS, Connections, etc.). Compile and organize appropriate client intake paperwork, surveys, and forms. Maintain casefiles, database entry, and record keeping in compliance with ACS, OCFS, and AAFSC requirements, in alignment with the Solution Based Casework Model guidelines (as needed). Appropriately follow incident reporting procedures (as needed). Maintain relationships with ACS personnel, including DCP workers, joint home visit ACS staff, and FSU workers, for ongoing ACS supervision. Regularly collaborate and communicate with PPRS staff by responding to emails, maintaining an up-to-date Outlook calendar, participating in supervision meetings, and scheduling office days. Perform other job-related duties as assigned. Trainings, Meetings, and Audit Preparation Attend training provided by ACS, Solution Based Casework, and other organizations as required (minimum of 42 hours/6 days each fiscal year) and log training by submitting certificates to the Director of Training and Development. Provide psychoeducation and information to enhance survivors' understanding around trauma-related effects, legal systems, immigration, and other systems especially as they relate to domestic violence. Provide support, education, referrals, safety planning and advocacy related to parenting in the context of domestic violence including educational settings, custody/visitation matters, the effects of DV on children, etc. Utilize best practices that engage survivors with culturally affirming frameworks promoting resilience, social connection, and self-efficacy. Make appropriate referrals for other services and provide follow-up. Co- facilitate a weekly support group. Document services and case information and coordinate grant reporting. Document and collect evidence of abuse that could support civil legal remedies and/or prosecutions. Participate in supervision, case planning conferences, statistical and narrative documentation, agency orientation, trainings and staff meetings. Collaborate with NMIC staff in the areas of family law, immigration, housing, benefits, education, employment to provide holistic services to survivors. Collaborate with partners and other community and city agencies in efforts to prevent intimate partner violence and respond to survivors' needs and right to self-determination. Participate in community outreach, awareness campaigns, special events and /or educational workshops as needed by DVP. Additional related duties may be assigned. JOB OBJECTIVE: To provide high-quality case planning, case work, and case management services to at-risk families referred by the Administration for Children’s Services. These families may be experiencing a wide range of challenges, including personal/marital/relationship problems, financial stressors, acculturation issues, family destabilization, and/or parent/child conflict. The Case Planner provides intensive preventive services to avoid foster care placement and ensure the safety and welfare of children in the home. He or she provides the family with services according to ACS standards and requirements. The Case Planner follows a Solution Based Casework (SBC) practice model. SBC is based on three theoretical foundations: family life cycle theory, relapse prevention/CBT theory, and solution-focused family therapy. PRIMARY DUTIES & RESPONSIBILITIES: Case Management & Advocacy Ensure timely completion of SBC training & certification. Conduct home visits and office visits to assess child safety and risk elements, complying with ACS requirements. Face-to-face contacts should be conducted twice per month, at a minimum and includes home visits (two home visits must be conducted if children 0-2 years old are present) and office visits (e.g., school, doctor’s office, AAFSC), as well as other out-of-home visits (e.g., park). More visits may be required. Provide case work/case management services to families receiving Preventive Services (12 cases); which includes but is not limited to providing supportive casework, focusing on service goals, and child welfare laws, improving family relationships and bonding, coping skills, parenting, etc. Ensure joint-home visits are conducted within 5 business days from the date of referral. Evaluate and assess the current family functioning including child safety and home safety (e.g., assessing other adults in the home, ensuring safe sleep, working fire alarms, window guards, etc.). Work in partnership with the family to formulate outcomes & service plans with the family. Ensure all initial intake documents (AAFSC intake, Domestic Violence screening, child trafficking) are completed and processed accurately using AAFSC internal database (i.e., Apricot). Advocate for the family by linking them to needed services in the community: medical, mental health, entitlements, day care, home health aide services, substance abuse treatment, anger management, etc. and follow up to monitor participation and progress. In collaboration with the Conference Facilitator, support the facilitation of Family Team Conferences and Service Termination Conferences, as well as other relevant ACS-led conferences. Family Team Conferences must be conducted within 4 weeks of the FASP due date. Ensure FASPs are launched 30 days prior to the due date and submitted to Supervisor 3 weeks prior to the due date. Complete Initial, Comprehensive, all Reassessment FASPs, and Plan Amendments for all Advocate Preventive Only (ADVPO) cases offline. We proudly lead advocacy efforts that rally our community to promote human rights, social justice, and economic opportunities for all. We are built upon the simple truth that people are experts in their own lives. Our programs are spread across 25 locations and include over 1200 units of supportive housing in three boroughs, the Howie the Harp Advocacy Center, Peer-driven Crisis Respite Center, Assertive Community Treatment (ACT) and the Intensive Mobile Team (IMT). Program Description Community Access’s OMH licensed Intensive Crisis Residence (ICR) is inspired by the Soteria model and is designed as an alternative to psychiatric hospitalization for people going through an acute psychiatric crisis and who would benefit from a respite period in a stabilizing, home-like environment with 24-hour peer support and treatment services. The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns. The ICR will follow a Soteria model which emphasizes shared running of a home-like environment, positive expectations of recovery, validation of subjective experiences, the concept of “being with” a person, providing 1:1 support, 24 hours a day, and self-determination. Services are completely voluntary, and guests are encouraged to maintain their daily responsibilities and activities in the community during their stay including work, school, family care, recreation, and wellness appointments. The program works with guests throughout their stay on discharge planning focused on safety and wellness, and provides post-discharge follow up health resource navigation support. Position Overview The Peer Respite Worker develops mentoring and supportive relationships with ICR guests, promoting connection and hope, and utilizing the stabilizing routines of the home-like environment to deescalate crisis and promote recovery-oriented practices. The Peer Respite Worker engages guests in individual and group-based peer support services to identify and develop crisis-management skills, addressing questions and fears, while building self-advocacy and systems-navigation skills. The Peer Respite Workers collaborate with the full ICR team to promote a welcoming, home-like atmosphere, and conduct group activities that foster connection, mutual understanding, and wellness. Job Description About the Program In this new Clinical Support Program, Rising Ground will embed licensed professionals specializing in mental health, substance misuse, and intimate partner violence directly into child protective work in Brooklyn. These clinicians will collaborate closely with ACS staff and families to assess needs, provide tailored service recommendations, and ensure families are connected to appropriate supports. By integrating clinical expertise into frontline decision-making, Rising Ground will play a vital role in enhancing child safety and strengthening family outcomes across the borough. About the Role The full-time Clinician provides direct clinical services and clinical guidance, training, and support to an ACS Child Protective Services team. The clinician works collaboratively with the CPS team, providing feedback on a family’s mental health needs, assisting with referrals for subsequent mental health treatment and providing warm hand offs to community based providers. This position requires daily in-person presence at an ACS field office in Brooklyn or in the community as needed. Occasional travel to other agency sites or meetings may also be required. Job Description The Licensed Supervising Social Worker plays a central role in maintaining the Safe Horizon Counseling Center (SHCC) as the first New York State-licensed outpatient mental health clinic to specialize exclusively in the evidence-based treatment of traumatized survivors of all ages who have experienced interpersonal violence. SHCC also functions as a resource to the agency with regard to trauma-focused treatment for survivors of crime. The Licensed Supervising Social Worker provides weekly supervision and training to a maximum of 7 staff therapists as well as social work and mental health counseling interns. Supervision includes providing weekly support to our treatment staff in reaching administrative as well as clinical productivity goals. This includes careful review of therapist documentation to ensure compliance with all city, state, and federal regulations as well as Safe Horizon productivity and practice requirements. The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns. Job Description Licensed Social Worker / Mental Health Clinician (1099) Location: Queens, Bronx, or Brooklyn, NY (Work in Your Preferred Borough) Pay: • LMSW, LMFT, LMHC: $75/hour, • LCSW: $100/hour Job Type: Independent Contractor (1099) Caseload: Approximately 20 clients per month We are a growing private psychotherapy practice specializing in services for individuals with intellectual and developmental disabilities (IDD) and co-occurring mental health conditions. We are seeking compassionate licensed clinicians to provide in-home psychotherapy services throughout their preferred borough—Queens, the Bronx, or Brooklyn. You will only travel within the borough you choose. Responsibilities • Conduct comprehensive psychosocial assessments and develop individualized treatment plans, • Provide individual, family, and caregiver counseling services, • Deliver evidence-based therapy tailored to individuals with intellectual and developmental disabilities, • Collaborate with behavior analysts, speech-language pathologists, occupational therapists, and other providers, • Educate and support families and caregivers throughout the treatment process, • Monitor client progress and modify treatment plans as needed, • Complete clinical documentation and progress notes within required timeframes, • Maintain compliance with Medicaid, Medicare, and private insurance documentation standards, • Participate in multidisciplinary care coordination, • Promote community inclusion and advocate for individuals with developmental disabilities Requirements • Active New York State LCSW, LMSW, LMHC, or LMFT license in good standing, • Experience working with individuals with intellectual and developmental disabilities or a strong interest in this specialty, • Excellent clinical, communication, and documentation skills, • Knowledge of New York State psychotherapy regulations and ethical standards, • Ability to work independently while collaborating with a multidisciplinary team, • Justice Center Incident Management and Mandated Reporter certifications preferred, • Valid driver's license and reliable personal vehicle required, • Must be comfortable driving throughout your selected borough (Queens, Bronx, or Brooklyn) to provide in-home services, • Strong organizational skills with the ability to manage a flexible caseload Bilingual candidates and applicants with lived experience of disability are strongly encouraged to apply. Job Description OBJECTIVES: Reporting to the Fee for Service Program Coordinator, the Fee for Service Clinician will provide outpatient mental health services to clients and their families in our MHOTRS Behavioral Health Clinic’s Fee for Service Program. This role is dedicated to enhancing emotional well-being and strengthening family dynamics through individualized therapeutic interventions. The position requires adherence to all state regulations and professional standards while fostering a safe, inclusive, and supportive environment for a diverse client population. DUTIES AND RESPONSIBILITIES: • Deliver therapeutic services such as individual therapy, collateral therapy, and group therapy to a total of 6-10 clients; children, adolescents, and their caregivers in our Fee for Service Program by using evidence-based, trauma informed treatment approaches., • Provide services on site at our East Harlem location and via secure telehealth platforms, based on the needs of clients Monday-Friday and Saturdays., • Assess and formulate clinical diagnoses, administer required clinical assessments, develop individualized treatment plans, and monitor progress toward established goals., • Coordinate case management by arranging care and resources, facilitating referrals, and providing advocacy support for clients and families., • Provide immediate crisis intervention and safety planning for clients experiencing acute emotional distress., • Maintain compliance with agency regulatory standards by ensuring accurate and timely documentation in the electronic health record (EHR), including assessments, treatment plans, progress and informational notes, and discharge planning., • Ensure program accessibility for clients by maintaining a schedule that accommodates evening hours and after-school/work appointments. SPECIFIC SKILLS: • Deep understanding of HIPAA, FERPA, and professional ethical guidelines., • Ability to provide services that respect diverse backgrounds and identities., • Excellent verbal and written communication skills for interacting with clients, families, and multidisciplinary teams., • Bilingual in English and Spanish is preferred but not required. QUALIFICATIONS: • Must hold a valid clinical license in the State of New York such as Licensed Mental Health Counselor (LMHC) or Mental Health Counselor with Limited Permit (MHC-LP), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or a doctoral degree in psychology (Ph.D. or Psy.D)., • Experience providing therapy to children, adolescents, and families., • Strong knowledge of evidence-based practices, crisis intervention, and telehealth delivery. COMPENSATION AND BENEFITS: Pay range: Per Diem $28-$55 commensurate on length of session provided.
