This program offers a range of services and supports directly accessible to families through partnerships with local schools. Position: Case Planner Reports To: Program Manager Location: 955 Flatbush Ave 4th Floor Brooklyn NY 11226/ Hybrid Remote What The Case Planner Does: • Conducts outreach activities to engage families, including those who self-refer, and discuss program offerings and expectations. This outreach prioritizes referrals from partner schools, encompassing educators, support staff, and school-based service providers (health and mental health clinics, after-school programs, community school lead organizations, etc.), and extends to the broader community-based organizations (CBOs) and internal CAMBA programs to ensure a holistic approach to family support., • Collaborates with referral sources, including schools (educators, support staff, and school-based service providers) and CBOs, to gain a comprehensive understanding of each family's unique strengths, needs, and priorities, ensuring a good program fit. In this collaboration, informed consent for information sharing is secured from families to facilitate service delivery., • Develops individualized support plans in partnership with families. These plans are based on comprehensive assessments that identify families' strengths, needs, and service requirements., • Manages a caseload of 12 clients effectively., • Maintains accurate documentation (FASP reports, databases)., • Prepares reports and participates in program evaluation., • Recommends case closure based on guidelines., • Participates in case conferences. Case management staff focusses on removing barriers to maintain medical care and achieve viral suppression by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focuses on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Position: Case Manager (Bilingual) Reports To: Assistant Program Manager Location: 19 Winthrop Street, Brooklyn NY 11225 What The Case Manager (Bilingual) Does: • The Case Manager will be responsible for providing all contract funded activities. The first program provides 70 units of housing to chronically homeless single adults who are eligible for SSI or SSD and suffer from Serious and Persistent Mental Illness (SPMI) or are diagnosed as Mentally Ill and Chemically Addicted (MICA). The second program provides 10 units of housing for SPMI individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes. The third program provides 7 units of housing to individuals who are SPMI and have been residents of either OMH Psychiatric Centers or OMH operated residential programs for one year or longer. All apartments are located in Brooklyn. CMS is the city-wide effort coordinated by the Mayor’s Office of to Prevent Gun Violence (OPGV) within the Mayor’s Office of Criminal Justice (MOCJ) to reduce gun violence and address its impact on communities. BIVO works with 16 – 24 year old at risk youths who are or may have potentially been involved in street violence, community residents, businesses and community-based organizations to implement the Cure Violence Model. The Cure Violence model is a community-based public health approach to preventing gun violence that is replicated in numerous large cities around the country and internationally. This program will focus on a defined area identified as a program service area in Brownsville, Brooklyn for shooting violence within the 73rd Precinct. The program works to prevent gun violence by: 1) mediating conflicts that may end in gun violence, 2) changing the mindset of individuals who are most at risk of being victims or perpetrators of violence, and 3) working closely with a broad coalition of local partners to promote a visible community-wide message that shooting is unacceptable behavior 4) helping organize Brownsville Safety Alliance resource fairs with the 73rd precinct to provide additional support to Brownsville residents.   Job Description Summary: Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach. Under this umbrella, Case managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). This position will focus on providing support and services to LGBT youth. Services provided include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Case manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Engagement of adolescents into program structure (harm reduction philosophy, rules, mutual respect), • Intake and assessment for individuals seeking assistance and screen for housing eligibility, • Provide case management to a caseload of LGBTQ youth, • Assist Youth in applying to Supportive housing and other permanent housing options, • Coordinate with Housing Navigators and Housing Advocate & Benefits Coordinator in follow up with clients and gathering of documentation for processing, • Provide ongoing support to youth in housing following exit of shelter/drop-in. The project has 121 residential dwelling units, one of which will be a superintendent's unit. The unit distribution is 46 studios, with the remainder being one and two bedroom units. CAMBA, Inc. is the on-site services provider at Via Vyse. Sixty one apartments, 51% of the units in the building, will be master leased by CAMBA and will house chronically homeless single adults and families who, the head of households, are diagnosed with chronic health conditions. The remaining 59 units are occupied by community residents, who may require some services. Position: Bilingual (Spanish) Case Manager Reports To: Program Manger Location: 1812 Vyse Avenue Bronx, NY 10460 What The Bilingual (Spanish) Case Manager Does: • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Conduct group sessions and/or workshops to assist tenants with independent living skills., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building), • May schedule appointments for tenant with referral organizations., • May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants' behalf., • May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • May evaluate actual living conditions of tenants through home visits. THE OPPORTUNITY Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court and community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in so doing, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. SIJC seeks a Part-time Case Manager for the borough’s Supervised Release Program (SRP). Reporting to Supervising Case Manager, the Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, Case Managers will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants’ compliance with program requirements. Accurate and timely data entry is a core responsibility for this position. Additionally, Case Managers will co-facilitate pro-social and psycho-educational groups.
