THE OPPORTUNITY Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court and community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in so doing, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. SIJC seeks a Part-time Case Manager for the borough’s Supervised Release Program (SRP). Reporting to Supervising Case Manager, the Case Managers are the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, Case Managers will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants’ compliance with program requirements. Accurate and timely data entry is a core responsibility for this position. Additionally, Case Managers will co-facilitate pro-social and psycho-educational groups. Office of Mental Health (OHM) Supported Housing Program: provides a total of 87 units of scattered site housing to individuals who meet the program’s eligibility. The first program provides 70 units of housing to chronically homeless single adults who are eligible for SSI or SSD and suffer from Serious and Persistent Mental Illness (SPMI) or are diagnosed as Mentally Ill and Chemically Addicted (MICA). The second program provides 10 units of housing for SPMI individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes. The third program provides 7 units of housing to individuals who are SPMI and have been residents of either OMH Psychiatric Centers or OMH operated residential programs for one year or longer. All apartments are located in Brooklyn. The program offers Case management services focusing on removing barriers to maintain housing by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focus on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Clients contribute 30% of their income towards their rent. Position: Case Manager Reports To: Program Supervisor Location: 19 Winthrop Street, Brooklyn NY 11225 What The Case Manager Does: • The Case Manager will be responsible for providing all contract funded activities. These activities include conducting and completing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conference, etc. In addition, the Case Manager will be responsible for completing all program documentation and entering all client services into to Client Track. PROGRAMMATIC OVERSIGHT Maintain an emotionally and physically safe environment for youth that meets all compliance and safety requirements set forth by the organization, DOE, DOH, funders and applicable labor law. Identify needs of the targeted students, apply interventions, and track and monitor input and impact of interventions for weekly meetings about student progress. Monitor and track daily, weekly, and monthly attendance, and follow-up with family, as needed, to ensure continuous engagement and attendance. Identify high need students, coordinate with teachers to target interventions, and track and monitor impact of interventions. Facilitate alliances and partnerships with City agencies and non-profit partners to establish a continuum of services for students. Support New York Edge, school, and partner agencies with resource management and development, including fund development and grant maintenance. Support the coordination of space for student and family support services, and for special events. Support program activities including but not limited to expanded learning time, parent family and community engagement, and mental health. Participate in school team meetings and specialized meetings such as attendance meetings. Facilitate the process to gain additional resources. RELATIONSHIP AND PARTNERSHIP MANAGEMENT Provide holistic assessment, creation, and implementation of a Family Development Plan (FDP) that outlines participants’ strengths and self-defined goals, specific steps to take to achieve goals, available supports and resources needed, and a realistic timeline to meet the goals. Regularly review the FDP to assess whether goals have been met or need to be adjusted mid-course, Provide follow-up to ensure that the participants have received requested services. Cultivate and forge positive relationships with Principal, Assistant Principals, teachers, parents and parent coordinators and other key constituents in the community to engage their support in ensuring programmatic quality for the youth we serve. Help broker new partnerships aligned with student goals and needs. Collaborate with all program partners on program delivery to align with the goals in relation to student achievement and performance. Establish parent involvement in areas of advocacy, special events, daily operations, education, and trips. Identify and utilize community resources, including other schools, libraries, community and local government agencies. Work effectively with internship sites. Conduct outreach to students and families. Keep supervisor informed of pertinent site information, including any challenges in fulfilling responsibilities Seek input from teachers, school staff, parents and students to determine student and family needs. Build relationships and mechanisms to link school day to extended day learning opportunities for targeted cohort. Participate in data collection, reporting and analysis. OPERATIONS Ensure the Program is compliant with DOE regulations, timelines, and staying abreast of all local, state and federal management conditions and requirements. Monitor and ensure all program compliance, including but not limited to student enrollment, attendance tracking, and online virtual learning. Ensure that student attendance information is being properly collected and delivered to the Data Department on a weekly basis. Maintain accurate records (i.e., staff files, program flyers, materials, schedule and curriculum). Report site injuries via appropriate forms and immediately communicate incidents with school and New York Edge personnel. Assist Program Manager in ensuring budgetary compliance and with submission of any required documents or reports to funding sources. Track and care for all equipment and supplies used by program participants and staff. Scattered Site Housing Program provides 270 scattered site-housing units to HIV/AIDS Services Administration (HASA) clients. All the units are leased in the agency’s name. Sixty-seven units are dedicated for families and 203 units are for single individuals. The program has two components to assist clients succeed in housing, socials services and operations. Case management staff focusses on removing barriers to maintain medical care and achieve viral suppression by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. The operations component focuses on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases, and making units ready when there is a vacancy. Position: Case Manager (Bilingual) Reports To: Assistant Program Manager Location: 19 Winthrop Street, Brooklyn NY 11225 What The Case Manager (Bilingual) Does: • The Case Manager will be responsible for providing all contract funded activities. These activities include conducting and completing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, Case Conference, etc. In addition, the Case Manager will be responsible for completing all program documentation and entering all client services into to Client Track. Will be responsible for ensuring clients are connected to and maintain primary medical care and adhere to medical and medication treatment., • Connect clients to needed services, such as, medical, mental health, substance use treatment, as well as, legal, entitlements, etc., • Monitor clients' progress toward their goals (dates achieved) via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via progress notes., • Function as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc., • Escort clients to appointments (educational, medical, social service, etc.), • Assist clients in completing applications for benefits and entitlements and monitors outcome of process., • Collect all required documentation and create client files., • Conduct a minimum one home visit per month., • Report any repair issues to the Housing Specialist., • Monitor clients’ living conditions., • Meet required monthly projections., • Provide all required information for the completion of monthly program reports., • Maintain clients’ charts always audit ready.
