We're seeking an experienced Community Care Physiotherapist to join a highly regarded community services team supporting older people and people living with disability locally in areas of Bayside and Glen Eira. This rewarding role offers the opportunity to work autonomously while collaborating with a multidisciplinary team to improve clients’ independence, mobility and quality of life. About the role As the Community Care Physiotherapist, you'll provide comprehensive physiotherapy assessments, treatment and rehabilitation programs that enable clients to remain living safely and independently in their own homes. You’ll also support community care staff by conducting functional capacity assessments and providing guidance on safe work practices and injury prevention. Key responsibilities • Conduct comprehensive physiotherapy assessments for older people and people living with disability., • Develop personalised treatment plans and tailored home exercise programs., • Monitor client progress and review interventions to achieve the best possible outcomes., • Support clients to maintain independence, mobility and confidence in their own homes., • Work collaboratively with clients, carers, families and multidisciplinary health professionals., • Identify appropriate referral pathways and connect clients with additional support services., • Undertake functional capacity assessments for community care staff., • Provide education and advice to support staff on exercise programs, manual handling and safe work practices., • Maintain accurate clinical documentation, care plans and assessment reports., • Participate in case conferences, team meetings and continuous improvement initiatives. About you You are a compassionate and experienced Physiotherapist who enjoys working independently while being part of a collaborative team. You'll bring: • Bachelor of Physiotherapy (or equivalent)., • Current AHPRA registration as a Physiotherapist., • Experience working in community or home-based physiotherapy., • A sound understanding of the aged care and community care sectors., • Strong assessment, clinical reasoning and care planning skills., • Excellent communication and relationship-building abilities., • The ability to work autonomously and manage a varied caseload., • Current Victorian Driver Licence., • Current First Aid and CPR certification (or willingness to obtain).
