Interprets the appropriate information needed to identify each patient’s individual needs. Provides professional nursing care under supervision, for assigned patients, according to established standards and practices. /p h3General Responsibilities /h3 ol liCollects referral appointment information for medical care as recommended by staff providers with outside medical providers; may assist patients in making appropriate appointments as necessary by the office or as requested by patients. /li liCompletes referral documentation to include verifying patient eligibility, address, third party coverage and cost of services. Assures that hard-copy referrals are complete and ready for patient pick up and sent to the appropriate office. /li liInforms and educates patients on referral procedure and obtains appropriate signatures on forms as needed. /li liMaintains referral logs and referral tracker system. /li liMaintains updated log of approved vendors and their addresses, phone and fax numbers. /li liObtains authorization from insurance carriers through their online portal, phone, or fax. /li liEngages and collaborates with outside agencies in an effective and professional manner. Assists with the coordination of health records (i.e. from Health Information, X-Ray Technician) required to be sent to the specialists prior to appointments. /li liOrders medical supplies and durable medical equipment that is not available on site. /li liInforms patients of any special instructions for testing prior to appointments. /li liMaintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards. /li liCooperates with other personnel to achieve department objectives and maintain good employee relations, interdepartmental objectives, and infection control policies. /li liDemonstrates sensitivity to patient's comfort and privacy and expresses willingness to aid patients appropriately. /li liDocuments all patient contacts according to the provisions of the Privacy Act of 1974. /li liEnsures accuracy in recording of patient health records. /li liParticipates in medical chart audits as directed by supervisor. /li liActs as liaison between patients and medical staff while completing the referral. /li liDocuments all no-show appointments for outside referrals in designated area in patient’s chart and also notifies referring provider. /li liWhere applicable, must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients of all ages. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient's requirements relative to their age‑specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures. /li liEnsures confidentiality of records according to the provisions of the Privacy Act of 1974 and HIPAA. /li liParticipates and cooperates in the re‑credentialing process with the Contracting and Credentialing Coordinator. /li liMust maintain all licensures and certifications of the position. /li liUpholds and promotes the value and integrity of the Seneca Nation Health System’s mission statement.
